What the NayaScore assessment measures

You cannot personalise what you have not measured. The NayaScore assessment is how we establish where you are starting from, in numbers you can see for yourself. It is taken before treatment begins and repeated during a course, so change shows up while there is still time to act on it.

Three things are measured. Grip strength, as a plain physical marker. Heart rate variability, recorded continuously by a wearable ring rather than sampled once in a clinic room. And cognitive testing across twelve domains, including attention, planning and response inhibition, reported as percentiles against people of your age and sex.

The NayaScore is a measurement tool, not a diagnostic test. It does not name a condition and it does not replace a clinical opinion. Your clinician reads it alongside your history, your symptoms and their own judgement, and will say plainly what the numbers can and cannot tell you.

What the assessment involves

Consultation & preparation

Sleep and eat as you normally would, and take your usual medication unless your clinician has said otherwise. Keep caffeine to your ordinary amount rather than skipping it or doubling it, since both distort cognitive scores. Bring your reading glasses if you use them for a screen. Book a time of day you could realistically repeat, because comparing a morning baseline with a late-afternoon retest introduces a difference that has nothing to do with treatment. Tell us about any hand or wrist injury before the grip test rather than during it.

Aftercare

Your clinician talks the results through with you rather than sending a score and leaving you to interpret it, and will be specific about which numbers are meaningful and which sit inside ordinary variation. The ring keeps recording between appointments. The assessment is repeated during your course so progress is measured against your own baseline, and your results are kept, so a later course starts from a real history rather than from memory.

NayaScore questions

Is the NayaScore a diagnostic test?

No. It is a measurement tool for tracking change over time, and it cannot name a condition or replace a clinical opinion. Your clinician reads it alongside your history, your symptoms and their own judgement. If what you need is a diagnosis, that is a separate conversation with an appropriate specialist.

How long does the assessment take?

Allow about an hour in the clinic for the grip testing and the cognitive tasks, plus time to fit the ring and explain what it records. The heart rate variability part then runs quietly in the background across your ordinary days. Your results are talked through with you at your next appointment.

What are the twelve cognitive domains?

A cognitive domain is a specific mental ability measured separately from the others. Attention, planning, response inhibition and mental rotation are four of the twelve tested here. Your clinician will name the full set at your assessment, and explain which of them matter most for the symptoms you came in with.

Do I have to wear the ring all the time?

Wear it as much as you comfortably can, including overnight, because sleep is where some of the most useful heart rate variability data comes from. Gaps are not a problem. If wearing it becomes uncomfortable, or you would rather not, say so, and the rest of the assessment still stands.

What if my scores do not improve?

Then you will know, and you will know early. That is the reason for measuring at all. Not everyone responds to stimulation, and repeating the same measures partway through a course means the answer arrives while decisions can still change. If the numbers are not moving, we will tell you and stop.
